Despite the penalties, players still get caught using steroids:

Mariners reliever David Rollins has been suspended 80 games for the use of performance-enhancing drugs, MLB announced Friday.

According to USA Today, Rollins was caught using Stanozolol, a muscle-building anabolic steroid.

Rollins admitted his use, calling it a “serious error in judgment”. Note that David never actually played in the majors, but was in Spring Training with the Mariners after being picked up in the Rule 5 Draft. He probably thought this was it as far as making the majors, despite excellent three-true outcome majors throughout his career.
